PASSPORT REQUIREMENTS

FOR AGES 16 AND OVER:
AGES 16 AND OLDER - PASSPORT GOOD FOR 10 YEARS
Must complete lines 1 thru 22 of passport application. Do not go past line 22. The remainder must be filled out and signed in our presence.

For ages 16 or 17, the child and parent must sign line 23 of the application.

If applicant is 16 or 17 years of age and has their own driver's license or any form of a pictured ID, they have to use it in Section 23. If they do not have one, the parent can use their ID for Section 23.

For ages 18 and older, only the applicant has to sign Section 23 of the application.

If applicant is 18 years of age or older, they must use their own form of Identification for Section 23.

Cost is check or money order made payable to U.S. Department of State In the amount of $75.00 AND cash, check or money order made payable to the Clerk of Court for processing paper work In the amount of $25.00. (Plus an additional $10.00 if you wish to have your passport photo taken by the Clerk of Court)

You must have two identical passport photos, which our office can take for you.

You must have a certified copy of your birth certificate or an old passport to send to Passport Services. (They will be returned to you)

Bring all of this into the Clerk of Court's Office for processing. It will take about six weeks to get your passport.


FOR AGES 15 AND YOUNGER: (BOTH PARENT MUST SIGN)
AGES 15 AND YOUNGER - PASSPORT GOOD FOR 5 YEARS (CANNOT BE RENEWED)

Must complete lines 1 thru 22 of passport application. Do not go past line #22. The remainder must be filled out and signed in our presence.

If the applicant is age 13 thru 15, the child and both parents must sign Section 23 of the application.
Cost is check or money order made payable to U.S. Department of State in the amount of $60.00 AND cash, check or money order made payable to the Clerk of Court for processing paper work in the amount of $25.00. (Plus an additional $10.00 if you wish to have your passport photo taken by the Clerk of Court)
You must have two identical passport photos, which our office can take for you.
You must have a certified copy of your birth certificate or an old passport to send to Passport Services. (It will be returned to you)
Bring all of this Into the Clerk of Court's Office for processing.


RENEWAL:
MUST HAVE BEEN 16 YEARS OF AGE OR OLDER WHEN ISSUED TO GET A RENEWAL

Cost is check or money order payable to U.S. Department of State for $55.00.
Complete the front of the renewal form
Date and sign the bottom of form
Must have two new passport photos (Clerk's Office will take for fee of $10.00)
Must present old passport (Must be less than 15 years old from Issue date)

EXPEDITE FEES:
Expedite fees must be money order payable to U.S. Department of State for $135.00.

U.S. PASSPORT CARDS: Valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean only.

First time applicants: $45.00 Adults and $35.00 for minors
